Just In case you haven’t yet given the district permission to share your child’s photo for our school yearbook, please fill out this form completely and return to the main office.

If you have more than one child, please note that there must be a form completed for EACH CHILD.

This will also provide permission if your child is included in event photos (field trips, Camp Timberlee, school-wide concerts & activities,  Club and enrichment activity group photos, etc.
